PMOS Support Stack

These are the ingredients most often discussed in PMOS research — not a substitute for the hormonal and metabolic workup and treatment plan your clinician would build with you, but a reasonable starting point to bring to that conversation.

Myo-inositol (40:1 myo:D-chiro ratio)

We use: Ovasitol (Theralogix)2 level scoops/day, mixed into water (4,000 mg myo-inositol + 100 mg D-chiro-inositol total)

The most-studied supplement for PMOS/PCOS — researched for improving insulin sensitivity, ovulation, and menstrual regularity. The 40:1 myo-to-D-chiro ratio matters: it mirrors the ratio naturally found in healthy ovarian tissue, and the published PCOS trials showing benefit used this specific ratio, not myo-inositol alone or a mismatched blend. We use Ovasitol specifically because it's the product most of those trials were actually built on, not a generic substitute at a lower dose — a half-dose version won't reproduce the studied effect. Can take 6–12 weeks of consistent use to show its full effect.

Vitamin D3

We use: Pure Encapsulations1,000–2,000 IU/day, more if a blood test shows you're deficient

Low vitamin D is common in PMOS and linked to insulin resistance. Worth checking your level with a blood test rather than guessing the dose — take with a meal containing some fat, since it's fat-soluble.

Omega-3 (EPA/DHA)

We use: OmegaGenics 5-in-1 Fish Oil (Metagenics)1,000–2,000 mg combined EPA/DHA per day (2 softgels/day of a 1,000mg formula)

Studied for its effect on inflammation and lipid levels, both often affected in PMOS. Take with food to reduce any fishy aftertaste or stomach upset.

N-acetylcysteine (NAC)

We use: Pure Encapsulations1,800 mg/day (3 x 600mg capsules)

Researched for insulin sensitivity and ovulation, sometimes used alongside or as an alternative to metformin. The clinical trials showing benefit for PCOS/PMOS (Fulghesu et al., Thakur et al., among others) used 1,800 mg/day — a lower, token dose won't match what was actually studied. Occasional mild stomach upset is the most common side effect; taking it between meals with a small snack can help.
