Vaginal Health Stack

This is separate from the Bladder Health stack — that one's about the urinary tract, this one's about vaginal flora balance. The evidence here is more mixed than in some of our other stacks, and we've tried to be upfront about exactly how mixed, rather than oversell it.

Lactobacillus rhamnosus GR-1 & L. reuteri RC-14 (oral)

We use: Fem-Dophilus (Jarrow Formulas)1 capsule/day (5 billion CFU each of GR-1 and RC-14)

The most-studied probiotic combination for vaginal flora — taken orally, but shown to colonize vaginally. We use Fem-Dophilus specifically because it contains only these two strains at the full studied concentration, rather than a diluted multi-strain gut-health blend that includes them alongside other organisms at a fraction of the dose. Evidence is genuinely mixed: some support for reducing yeast colonization and helping maintain flora balance day to day, but a 2021 randomized trial found it didn't improve cure rates when added to antibiotic treatment for an active BV episode. Worth thinking of as flora maintenance, not active-infection treatment.

Vaginal probiotic suppository (L. crispatus-predominant)

We use: V-Probiotics (Vitanica)1 suppository vaginally at bedtime

Contains L. crispatus, L. gasseri, L. jensenii, L. reuteri, and L. rhamnosus. L. crispatus dominance is increasingly considered the single strongest marker of a healthy vaginal microbiome in current research — arguably more directly relevant than the oral GR-1/RC-14 combination above, though the two work differently (direct vaginal delivery vs. oral colonization) and can reasonably be used together. Mild discharge as it dissolves overnight is normal; avoid use during your period if possible.

Boric acid (vaginal suppository)

600 mg inserted vaginally at bedtime, typically for 14–21 days — this is a targeted regimen for recurrent infections, not a daily general-use supplement

Used for recurrent bacterial vaginosis (often after an oral antibiotic course, per CDC treatment guidelines) or recurrent/non-albicans yeast infections that haven't responded to standard antifungals. Only ~6% is absorbed systemically when used vaginally as directed.

Worth knowing

Boric acid capsules are for vaginal use only — never take them orally, since boric acid is toxic (potentially fatal) if ingested. They're also not safe during pregnancy. Keep them clearly labeled and stored away from anything that could be confused with an oral supplement.
