Hormonal Skin and Hair Changes: When Dermatology and Gynecology Overlap
A lot of skin and hair concerns that show up in a dermatology office have a hormonal driver that dermatology treatment alone doesn't address — clearing the skin without ever asking why the skin is doing this in the first place. Acne and hirsutism: the androgen-excess pattern
Persistent jawline/chin acne past the teenage years, combined with excess hair growth in a male-pattern distribution (upper lip, chin, chest, lower abdomen), is a classic androgen-excess presentation — most commonly PMOS (formerly known as PCOS). Treating the acne topically or with antibiotics can help the skin without ever addressing why androgen levels are elevated in the first place, which is a hormonal question, not a skincare one.
First-line hormonal treatment for both symptoms together is usually combined oral contraceptives, sometimes with an anti-androgen like spironolactone added specifically for hirsutism, since that responds more slowly and more partially to hormonal treatment alone than acne does.
Acanthosis nigricans: a skin sign of insulin resistance
Dark, velvety, thickened skin in body folds — the back of the neck, armpits, groin — is a visible marker of insulin resistance, not a hygiene issue and not treatable by scrubbing it away. It's strongly associated with PMOS and with type 2 diabetes risk more broadly, and its visibility makes it a genuinely useful early screening cue rather than just a cosmetic finding.
Hidradenitis suppurativa has a real hormonal link
Hidradenitis suppurativa — painful, recurring lumps in the armpits, groin, and under the breasts that can rupture and scar — flares for many people before their period and improves during pregnancy and after menopause, and it's associated with a meaningfully higher rate of PMOS. Dermatology manages the skin disease itself, but the hormonal pattern is worth naming directly rather than treating each flare as an isolated skin event.
Androgenic hair loss and postpartum hair loss are different things
Gradual thinning at the crown or part line, alongside acne or irregular periods, points toward the same androgen-excess pattern as above. Sudden, diffuse shedding starting a few months after childbirth is a different, temporary process (telogen effluvium) tied to the normal hormonal drop after delivery, and it typically resolves on its own within about a year — it's a different mechanism from androgenic hair loss even though both show up as "losing hair."
Melasma: a different hormone, a different mechanism
Melasma — symmetric brown-to-gray patches, usually on the face — is driven by estrogen and progesterone rather than androgens, which is why it shows up with pregnancy ("the mask of pregnancy"), combined hormonal contraception, and hormone therapy. It often fades on its own within months of delivery or stopping the hormonal trigger, though it can be slow to resolve and sun exposure keeps triggering it in the meantime — daily broad-spectrum sunscreen is genuinely central to management here, not just a general skin-health suggestion.
