Hot Flashes Without Hormones: What Actually Works

Hormone therapy remains the single most effective treatment for vasomotor symptoms (the clinical term for hot flashes and night sweats), but it isn't right or available for everyone — a personal or strong family history of certain cancers, a clotting disorder, or just personal preference are all valid reasons to look elsewhere. "Elsewhere" used to mean weak herbal remedies and hoping for the best; that's changed. Several genuinely evidence-backed non-hormonal options now exist, and one — fezolinetant — was designed specifically for this and approved in 2023.

Who this is actually for

Non-hormonal treatment is the right starting point if you have a personal history of breast cancer or another hormone-sensitive cancer, an unprovoked blood clot or stroke, or if you and your provider have decided hormone therapy isn't the right fit for you for any other reason — including simply preferring not to use it. It's also reasonable if your hot flashes are mild-to-moderate and you'd rather try a lower-intensity option first.

SSRIs and SNRIs

Low-dose paroxetine (7.5mg, brand name Brisdelle) is the only medication in this class specifically FDA-approved for hot flashes, but venlafaxine and escitalopram are also commonly used off-label with real supporting evidence, and can be a reasonable first choice if you're also dealing with mood symptoms during this transition. The effect size is real but more modest than hormone therapy — typically a meaningful reduction in frequency and severity, not full elimination.

One important interaction: paroxetine and fluoxetine both inhibit the liver enzyme (CYP2D6) that activates tamoxifen, meaningfully reducing its effectiveness — if you're on tamoxifen for breast cancer, venlafaxine or escitalopram are the safer SSRI/SNRI choices instead.

Gabapentin

Originally an anti-seizure medication, gabapentin has real evidence for reducing hot flash frequency, particularly at night — its sedating effect is a drawback during the day but genuinely useful for night sweats disrupting sleep, so it's often dosed specifically at bedtime. Dizziness and drowsiness are the main side effects to expect, especially when starting.

Fezolinetant (Veozah)

Approved by the FDA in 2023, this is the first medication designed specifically for hot flashes rather than repurposed from another use. It works by blocking a specific brain receptor (neurokinin 3) involved in the temperature-regulation pathway that goes into overdrive as estrogen declines — a genuinely different mechanism from anything above. Clinical trials showed meaningful reductions in both frequency and severity. It requires periodic liver enzyme blood test monitoring (at baseline, and periodically after starting), and isn't appropriate if you have active liver disease.

Non-drug measures

Cognitive behavioral therapy specifically adapted for menopause has real evidence for reducing how bothersome hot flashes feel, even when it doesn't reduce how often they happen — worth knowing if the disruption/distress is the main problem for you. Layered clothing, lowering the bedroom temperature, and identifying your personal triggers (alcohol, caffeine, and spicy food are common ones, though triggers vary person to person) are low-risk and worth trying regardless of what else you use. Weight management has modest supporting evidence too, though it's a longer-term strategy, not a quick fix.

Worth discussing with your provider

Whether a personal or family cancer history, clotting history, or preference is steering you away from hormone therapy — that shapes which non-hormonal option fits best
Any tamoxifen use, before starting an SSRI/SNRI specifically
Whether night-time symptoms specifically (vs. all-day symptoms) point toward a bedtime-dosed option like gabapentin
Baseline liver function testing if fezolinetant is being considered
