Twin and Multiple Pregnancy: What's Actually Different
A twin or higher-order pregnancy isn't just "a singleton pregnancy, times two" — the monitoring is more frequent, certain risks are genuinely higher, and one specific detail determined early on shapes almost everything about how closely you're watched. Here's what that detail is, and what else is actually different.

How twins happen, briefly
Identical (monozygotic) twins come from a single fertilized egg that splits into two; fraternal (dizygotic) twins come from two separate eggs fertilized separately, and are genetically no more alike than any other siblings. Chorionicity is the detail that actually drives your care
An early ultrasound (ideally in the first trimester, when it's easiest to tell) determines chorionicity — whether the twins share a placenta and/or an amniotic sac. Dichorionic-diamniotic twins each have their own placenta and sac, and carry the lowest additional risk. Monochorionic-diamniotic twins share one placenta but have separate sacs, and need closer monitoring because of that shared blood supply. Monochorionic-monoamniotic twins share both a placenta and a single sac — this is rare and carries the highest risk, requiring the closest monitoring of the three. This classification, not simply "having twins," is what actually determines how intensively your pregnancy is monitored from here on.
More frequent monitoring across the board
Expect more ultrasounds than a singleton pregnancy — growth is tracked separately for each baby, since it's common for the two to grow at somewhat different rates, and catching real discordance early matters. Prenatal visits also tend to start earlier and happen more often, especially as the pregnancy progresses.
Twin-specific risks worth knowing about
This isn't a list to worry over — it's context for why the extra monitoring exists. Preterm birth is meaningfully more common with twins than with a singleton pregnancy. Twin-to-twin transfusion syndrome (TTTS) is specific to monochorionic twins — an imbalance in blood flow through the shared placenta that sends too much blood to one twin and too little to the other — which is a major reason monochorionic pregnancies are watched more closely than dichorionic ones. Growth discordance between the two babies, and higher rates of preeclampsia and gestational diabetes compared to singleton pregnancies, are also more common with twins.
Delivery isn't automatically a C-section
Vaginal delivery is genuinely possible with twins and depends largely on the position of both babies, not a fixed rule that twins always mean a C-section. Delivery timing is often somewhat earlier than with a singleton pregnancy, but the specific timing is individualized to your situation and chorionicity rather than one universal date — this is a conversation to have with your provider as you approach the third trimester, not something to assume either way.
