Fertility Evaluation Profile
A fertility evaluation looks at a few distinct areas: whether you're ovulating, your egg supply, whether your tubes are open, the health of your uterus, your hormones, and — when applicable — your partner's contribution.
This questionnaire walks through your history — the same questions I'd ask in a first visit — and then shows you which of those areas may be worth evaluating, and how soon.
